Scaffolding failures cause dozens of fatalities every year — and most are preventable. Use this 12-point inspection checklist before every shift to ensure your scaffold is structurally sound, properly planked, and guardrails are in place.

Scaffolding is consistently one of OSHA's top-10 cited violations in construction — and scaffolding failures kill dozens of workers every year. The most common cause isn't bad equipment. It's skipped inspections and shortcuts taken by people who are in a hurry. This checklist eliminates the guesswork.

The 12-Point Pre-Shift Scaffold Inspection

  • 1Base plates and mud sills are in place and level on stable ground
  • 2All frames and cross braces are properly connected and locked
  • 3Planking fully covers the working platform with no gaps over 1 inch
  • 4Planks extend 6-18 inches beyond supports at each end
  • 5Top rail (38-45 inches), mid rail, and toe boards are in place
  • 6Access ladder or stairway is properly positioned and secured
  • 7Scaffold is plumb and level — check visually and with level
  • 8No signs of damage, corrosion, or deformation on any component
  • 9Capacity placard is visible and load limits are not exceeded
  • 10Overhead protection is in place if work is performed above
  • 11Competent person has reviewed the scaffold condition
  • 12Tag system shows current inspection status (red/green tagging)

Tip: Tag It: Use a scaffold tagging system — green tag means the scaffold has been inspected and is safe to use; red tag means it's been taken out of service. Workers should never use a scaffold without a green inspection tag.
